At the Foot & Ankle Health Center, the physicians and staff take their time to get to know each individual patient’s needs and goals. It is important that you fill out our questionnaire so that your doctor can fully understand your medical background. During your first appointment, your doctor will typically obtain a complete history of your injury, as well as perform a physical exam. If you have any recent diagnostic imaging (x-rays, MRIs, etc.), they will also review those images and reports with you.

Completed registration forms and questionnaire
Insurance card and photo ID so we can make a copy for our records
IF AVAILABLE:
- CD of x-rays or MRI images, along with corresponding reports of injured body part
- Previous operative reports of injured body part
- Notes from other doctors, chiropractors or physical therapists
CANCELLATION POLICY
We are sure to spend quality time with each patient. To ensure that we fulfill the needs of all our patients, please familiarize yourself with the cancellation policy of your doctors practice. Otherwise you will be charged fees for a no show or late cancellation.
